Consent form requirements

If your proposed research study requires participant recruitment/consent, you must follow the steps below to ensure compliance with British Columbia's Freedom of Information and Protection of Privacy Act (FIPPA) regarding the collection, use, linkage, and/or disclosure of personal information for research and evaluation purposes.

 

Step 1

Consult your Research Ethics Board (REB) for the appropriate consent form template.


Step 2

The BC Ministry of Health now requires a Consent Form Attestation to be completed: please see BC MoH Letter to Researchers. Refer to page two of the Consent Form Attestation and ensure that your consent form meets all the requirements listed.


Step 3

Obtain approval for your consent form from your REB.


Step 4

At the time of your DAR submission, you will need to:

  1. Provide a copy of your consent form that was approved by your REB.
  2. Ensure that your DAR includes the information requested on page 2 of the Consent Form Attestation.
  3. Provide a complete and signed copy of the Consent Form Attestation.
